New Patients:

If you are a new patient or are considering treatment at our office, Welcome!  If you would like, you can save yourself some time at your initial appointment by downloading and completing the new patient forms included below.  If you choose not to do this, please be advised that you will need an extra 15 or 20 minutes prior to your visit to complete this mandatory paperwork.

Scheduling:

Appointments are available Monday, Tuesday, and Thursday from 8 a.m. to 7 p.m., Wednesday from 8 a.m. to 5 p.m. and Friday from 8 a.m. to 1 p.m. With four highly skilled hygienists and a large enough office there is never a problem scheduling multiple appointments for you and your family.  So go ahead, bring the whole gang at the same time!

As for making your appointment(s), you will be happy to know that at Dr. Bracker’s office you will always get a live person to answer your call during business hours (Seriously, no automated systems whatsoever).
